I’m unable to send text messages through my business number while waiting for my registration to be processed due to the new telemarketing act. So I’ve redesigned my waiting list procedure so all communication will be through my electronic medical records platform, TheraPlatform. Here’s the new procedure…
- Click here or on the button on your screen to create an account in TheraPlatform and add your family to my waiting list.
- I’ll send you a quick waiting list form with only 2 questions (your insurance company & your scheduling needs) – sign back into TheraPlatform to complete this.
- Wait for a spot. While you’re waiting keep an eye out for emails from TheraPlatform. I’ll send you a message through there once a spot is available.
- Once you get a spot that works for you I’ll send you intake forms for our evaluation. These give me permission to evaluate your child and an idea of what kind of tests to use during our first appointment.
- Once you’ve completed your intake forms we’ll schedule our first appointment. Please note that failure to complete the forms within a week will result in your removal from the waiting list.
- Attend our first appointment. This will give me a chance to start getting to know your child and gather test results that will show whether insurance will pay for your child’s treatment.
